Welcome to our mobile and online application, lifelineTM (the “App” and the “Site”, respectively).

Your compliance with these terms of the Privacy & Security Policy (the “Terms of Use”) is a

The lifelineTM application (herein, “lifelineTM” or “the App”) is a product offering of RYKOV,

LLC (herein, “RYKOV”). LifelineTM is comprised of various application interfaces of a mobile

device and web pages operated by RYKOV. Your compliance with these terms and conditions

with Online Privacy and Security (“Terms of Use”) is a condition to your use of the App and/or

Site. The App is offered to you conditioned on your acceptance without modification of the terms,

conditions, and notices contained herein (the "Terms "). Your use of the App constitutes your

agreement to all such Terms. Please read these terms carefully and keep a copy of them for your

reference.

RYKOV, or any of RYKOV’s affiliates, is committed to providing you with your access, control

and ownership of YOUR HEALTH DATA by way of lifelineTM

. The App is built, in part, on our

commitment to respect the privacy and confidentiality of your medical information as well as your

personally identifiable information (PII). RYKOV has put into place detailed policies and

procedures regarding access to health records and data by our staff and employees, other users,

communities, and professionals that support the development of lifelineTM. We have also carefully

outlined the circumstances under which your medical information and health content and/or data

may be released to parties outside the organization in compliance with state and federal laws.

LIFELINETM follows the rules that apply under HIPAA and the HITECH Act, as well as any other

privacy and security obligations applicable to your personal information and obligations agreed to

between LIFELINETM and your healthcare provider, if appropriate.

Protecting your private information is our top priority. This Privacy and Security Policy applies to

RYKOV and lifelineTM, and governs data collection and usage. For the purposes of this Privacy

Policy, unless otherwise noted, all references to RYKOV include https://lifeline.global and the

mobile access to the App and the Site. The App allows you access and control of your health data

and access to other users within health communities. By using the App, you consent to the data

practices described in this statement.

If you have any questions regarding lifelineTM, your account or any other question, please

additional questions related to content of your electronic health records (EHRs) pulled in from

your health care provider through whom you obtained access to your EHRs directly, please contact

the health care provider. The content of lifelineTM, including without limitation, the files,

documents, text, photographs, images, audio, and video, and any materials accessed through or

made available for use or download through lifelineTM (“content”) may not be copied, distributed,

modified, reproduced, published or used, in whole or in part, except for purposes authorized or

approved in writing by RYKOV.

THE CONTENT OF LIFELINETM, INCLUDING WITHOUT LIMITATION, TEXT, COPY,

AUDIO, VIDEO, PHOTOGRAPHS, ILLUSTRATIONS, GRAPHICS AND OTHER VISUALS,

IS FOR INFORMATIONAL PURPOSES ONLY AND DOES NOT CONSTITUTE

PROFESSIONAL MEDICAL ADVICE, DIAGNOSIS, TREATMENT OR

QUALIFIED HEATH CARE PROFESSIONALS WITH ANY QUESTIONS OR CONCERNS

YOU MAY HAVE REGARDING YOUR INDIVIDUAL INFORMATION, NEEDS AND ANY

CONTENT PROVIDERS, EMPLOYEES OR MEDICAL STAFF RELATED TO THOSE EHRs,

EMPLOYEES OR STAFF SUPPORTING LIFELINETM, VISITORS TO LIFELINETM OR

OTHERS, IS SOLELY AT YOUR OWN RISK. NEVER DISREGARD MEDICAL ADVICE

OR DELAY IN SEEKING IT BECAUSE OF SOMETHING YOU HAVE READ ON

LIFELINETM.

YOUR PERSONAL INFORMATION, USERNAME & PASSWORD

Your privacy is important to us. You agree to use a personal username and create a password to

access and use lifelineTM. LifelineTM will store your personal information from multiple data

sources as authorized by you; your personal information including, for example, certain health

record and medical information from visits to healthcare providers. All content of the App and the

Site, includes but not limited to design, text, software, technical drawings, configurations,

graphics, other files, and their selection and arrangement (“Content”). Your electronic health

record (EHR) and medical expense information is not Content as defined in these Terms of

Use. All other information, images, these Terms of Use and logon information is Content.

Your Account and Access to LifelineTM

You are responsible for maintaining the confidentiality of your username and password and for

restricting access to your mobile device, computer, or other electronic device and lifelineTM

. You

agree not to share or disclose your username or password to any third party. You agree that

you are fully responsible for all activity occurring in your account or by any use of your username

or password. The administrators of lifelineTM reserve the right to refuse access or service, terminate

accounts, remove or edit content in their discretion. You understand that you are financially

responsible for all uses of the lifelineTM by you and those inappropriately using your username and

password information. You acknowledge that your access to lifelineTM may be revoked at any time

for any reason or no reason.

Children Age 12 -17

As required by the Children’s Online Privacy Protection Rule (COPPA), lifelineTM is notifying

you that it collects information online from children ages 12-17 (minors under the age of 12 are

protected by COPPA). Children ages 12-17 may use lifelineTM to access their electronic health

record (EHR) through third party integrations with the App. These minors may use features of

lifelineTM to access his/her health records, ask questions through community forums, post and

obtain information through activity feeds. If the minor has consent from parents/guardians to an

EHR account then the minor will provide personal identifying information, including an email

address. Personal identifying information is not provided to third parties.

The App retains personal information collected from minors only as long as reasonably necessary

to update the minor's account and health care information. Upon request of the parent/guardian,

and as required by COPPA (Children’s Online Privacy Protection Rule), lifelineTM administrators

trademark license.

Children Under Age 12

RYKOV does not knowingly collect personally identifiable information (PII) from children under

the age of 12. If you are under the age of 12, you must ask your parent or guardian for permission

to use this application.

Children under the age of 12 are not permitted to use lifelineTM and should not attempt to register

or provide any personal information to lifelineTM

If lifelineTM administrators determine that it has collected personal information from a child under

the age of 12, lifelineTM administrators will promptly delete that information. If you believe

lifelineTM has collected information provided by a child under the age of 12, please call contact

your health center.

Collection and Use of your Personal Information

In order to better provide you with products and services offered, RYKOV via the App, may collect

personally identifiable information, such as your:

If you purchase RYKOV products and/or services, we may collect billing and credit card

information. This information is used to complete the purchase transaction.

RYKOV may also collect anonymous demographic information, which is not unique to you, such

as your Age.

We do not collect any personal information about you unless you voluntarily provide it to us.

However, you may be required to provide certain personal information to us when you elect to use

certain products or services. These may include, but are not limited to: (a) registering for an

account; (b) entering a sweepstakes or contest sponsored by us or one of our partners; (c) signing

up for special offers from selected third parties; (d) sending us an email message; (e) submitting

your credit card or other payment information when ordering and purchasing products and

services. To wit, we will use your information for, but not limited to, communicating with you in

relation to services and/or products you have requested from us. We also may gather additional

personal or non-personal information in the future. We use your personal information to process

credit card / debit card transactions, and to exchange information with health care providers,

physicians, payers and other third parties that help us provide lifelineTM content to you. For

example, we may use third parties to help host and maintain the App, and to process payments.

The App may collect identifiable information about individuals who use its services in the

trademark license.

E-mails — The App may use your email address to send you notifications. You may change your

communication preferences at any time.

SMS — The App may use your phone number to send text messages related to your relationship

with RYKOV, including updates related to your visits, the App account, one-time passcode, billing

notifications, prescription reminders, and care management.

You may change your communication preferences at any time. You can opt-out of SMS messages

by texting STOP in response to a RYKOV or lifelineTM text message. Your opt-out request will

generate one final message confirming that you have been unsubscribed. You will no longer

receive SMS messages from the short code you opted out from. If you want to join again, sign up

using lifelineTM or text HELP to the short code for instructions.

The standard text message and data rates may apply depending on your mobile plan. Carriers are

not liable for delayed or undelivered messages. Message frequency may vary.

Messages — The App may contain forms for comments, questions, messages and referrals to

certain services. Messages sent through the App may become part of your medical record.

Location Services — The App may use your location data. Such information may be used, for

exemplary purposes and not limitation, to notify front desk staff when you arrive for an

appointment or to suggest healthcare providers that are near you, or even suggest events, forums,

or connections near you. The location services also assist in generating account usernames and

may assist with alias creation. LifelineTM will store your location data to connect you to health

communities within the app and find providers/physicians as to your use and search inquiries. You

can always change your location preferences on your device.

Camera — LifelineTM may use your camera to take new photos or to capture and transmit video

for video visits. You choose if you want to use photos to personalize your account or send them as

file attachments when you send messages to your healthcare providers. You are responsible for the

content you share on the App. Be mindful of sharing personal information, photos, and

identification.

Microphone — LifelineTM may use your microphone to capture audio for video visits. LifelineTM

will not store your audio data.

Storage — LifelineTM may access your device's storage to read and write files and photos you

choose to use in LifelineTM. These files and photos may be used as attachments that are sent to

your provider or they may be created from attachments sent to you from your provider. These files

or photos may also be used to set your profile photo in LifelineTM

Bluetooth – LifelineTM may access your device's Bluetooth to detect other nearby devices. This

information will be used to notify front desk staff when you arrive for an appointment or allow

you to connect to a physician’s office or other health provider office. LifelineTM will not store

trademark license.

Phone Call – LifelineTM may use your phone to call phone numbers displayed in the App.

LifelineTM will not store your call history or other call data.

NOTE: LifelineTM provides you access to your health data, control and ownership of your data.

Once received, you control who can access your data, who you share your information with,

and where it is shared and stored. Different communities and groups within the App allow you

to identify yourself, or communicate anonymously, respectively. DO NOT SHARE YOUR

IDENTITY or PII, or any personal information that you would not want communicated within

the anonymous content of the App.

Health and Fitness Platforms -- Apple HealthKit and Google Fit integration enables the

voluntary sharing of certain medical information from your device to lifelineTM. Answering

questionnaires and surveys about your health conditions enable voluntary sharing of information

about your health status or condition. Any shared data and questionnaire answers are not reviewed

or monitored on any routine basis. Any received data and questionnaire answers may be evaluated

during appointments or other encounters determined by the provider. Shared data cannot be used

to determine an emergency – if at any time you feel you are experiencing a medical emergency,

please call 911.

For Android Users – Required Google Play Disclosures for Certain Health Apps

LifelineTM was not created specifically for the COVID-19 pandemic. LifelineTM demonstration

existed before the COVID-19 pandemic to allow users potential access to their health information

on file with EHR systems. LifelineTM and RYKOV permits users to access COVID-19 related

vaccination information, laboratory test results, and documents with illness-related information

maintained by your health care organization. You may choose if or how you want to access,

display, or use COVID-19 data, similar to how you use other health related information to make

decisions about other conditions, services, tests, or vaccinations.

LifelineTM accesses, uses and shares your information as stated in the above section titled

“Collection and Use of Your Personal Information.”

LifelineTM may interact with your sensitive data to provide certain features, such as video

conferencing or mobile appointment check-in. The first time you try to use any of these features,

we will ask for your consent within the app and will only allow you to use a feature if you give

consent. You do not have to provide consent if you do not want to allow LifelineTM to interact with

your data as requested. LifelineTM is developed by RYKOV LLC; please refer to RYKOV’s Terms

and Conditions in combination with EULA and Privacy Agreements for more detailed information

about the limited ways they may interact with your information.

Use of Your Personal Information

RYKOV collects and uses your personal information to operate and deliver the services you have

requested. You as the user own your personal data, your health data, and have discretion to import,

share, or export data as you direct.

The App may also use your personally identifiable information (PII) to inform you of other

trademark license.

PERMITTED USE OF LIFELINETM

LifelineTM grants you access to make personal use of the App and not to download (other than

page caching) or modify the App, or any portion of it, except with tools provided by the App or

with the express written consent of RYKOV LLC. You are not permitted to sell access to lifelineTM

or any of its contents. You are not permitted to make or establish a derivative use of lifelineTM or

its contents; or download or copy any information for the benefit of another party, except as may

otherwise be allowed by appropriate use of lifelineTM (e.g. parent/guardian/minor relationship).

Use of data mining, robots, or similar data gathering and extraction tools is prohibited. LifelineTM

or any portion of it may not be reproduced, duplicated, copied, sold, resold, visited, or otherwise

exploited for any commercial purpose.

THIRD PARTIES AND EXTERNAL WEBSITES

Content. Third party health care providers, payers, publishers and distributors may own or make

available parts of the Content found on lifelineTM. LifelineTM is not responsible for third party

content or the privacy practices of the third parties. We recommend that you review the privacy

and security policies of such third parties prior to use of their Content. Any questions, complaints,

or claims related to any such information, services or products should be directed to the appropriate

third party.

Links and Third Party Websites. LifelineTM makes no representations whatsoever about any

other website that you may access through lifelineTM via a web link or otherwise. When you access

a non- lifelineTM site, please be advised that it is independent and separate from lifelineTM, and that

lifelineTM has no control over any content on that third party website. In addition, a link to a non-

lifelineTM website does not mean that lifelineTM endorses or accepts any responsibility for the

Content, or the use, of the linked site. LifelineTM will not be responsible for the information

contained in any third party website or for your use of or inability to use such website. Access to

any third party website is at your own risk, and you acknowledge and understand that a linked

third party website may contain terms and privacy policies that are different from those that apply

to lifelineTM

Sharing Information with Third Parties

RYKOV does not sell, rent or lease its customer lists to third parties.

RYKOV may, from time to time, contact you on behalf of external business partners about a

particular offering that may be of interest to you. In those cases, your unique personally identifiable

information (e-mail, name, address, telephone number) is transferred to the third party. RYKOV

may share data with trusted partners to help perform statistical analysis, send you email or postal

mail, provide customer support, or arrange for deliveries. All such third parties are prohibited from

using your personal information except to provide these services to RYKOV, and they are required

to maintain the confidentiality of your information.

RYKOV may disclose your personal information, without notice, if required to do so by law or in

the good faith belief that such action is necessary to: (a) comply with applicable law, regulation,

trademark license.

of potential violations; (c) detect, prevent, or otherwise address fraud, security or technical issues;

(d) protect and defend the rights or property of RYKOV, associated rights in lifelineTM, or features

our users, patients, health professionals, or the public as required or permitted by law.

Opt-Out of Disclosure of Personal Information to Third Parties

In connection with any personal information that we may disclose to a third party for a business

purpose, you have the right to know:

• The categories of personal information that we disclosed about you for a business purpose.

You have the right under the California Consumer Privacy Act of 2018 (CCPA) and certain other

privacy and data protection laws, as applicable, to opt-out of the disclosure of your personal

information. If you exercise your right to opt-out of the disclosure of your personal information,

we will refrain from disclosing your personal information, unless you subsequently provide

DISCONTINUING USE OF LIFELINETM

You may discontinue your use of lifelineTM at any time deleting the Application from your mobile

Right to Deletion

Subject to certain exceptions set out below, on receipt of a verifiable request from you, we will:

• Delete your personal information from our records; and

• Direct any service providers to delete your personal information from their records.

Please note that we may not be able to comply with requests to delete your personal information

if it is necessary to:

• Complete the transaction for which the personal information was collected, fulfill the terms

of a written warranty or product recall conducted in accordance with federal law, provide

a good or service requested by you, or reasonably anticipated within the context of our

ongoing business relationship with you, or otherwise perform a contract between you and

us;

• Detect security incidents, protect against malicious, deceptive, fraudulent, or illegal

activity; or prosecute those responsible for that activity;

Disconnecting your RYKOV Account from Third Party Websites

You will be able to connect your RYKOV account to third party accounts. BY CONNECTING

YOUR RYKOV ACCOUNT TO YOUR THIRD-PARTY ACCOUNT, YOU ACKNOWLEDGE

AND AGREE THAT YOU ARE CONSENTING TO THE CONTINUOUS RELEASE OF

INFORMATION ABOUT YOU TO OTHERS (IN ACCORDANCE WITH YOUR PRIVACY

SETTINGS ON THOSE THIRD PARTY SITES). IF YOU DO NOT WANT INFORMATION

ABOUT YOU, INCLUDING PERSONALLY IDENTIFYING INFORMATION, TO BE

SHARED IN THIS MANNER, DO NOT USE THIS FEATURE. You may disconnect your

account from a third-party account at any time. Users may learn how to disconnect their accounts

from third-party websites by visiting their “Settings” on lifelineTM

. Users may also contact us via

email or telephone.

Opt-Out & Unsubscribe from Third Party Communications

We respect your privacy and give you an opportunity to opt-out of receiving announcements of

certain information. Users may opt-out of receiving any or all communications from third-party

partners of RYKOV by contacting us here:

From time to time, RYKOV may contact you via email for the purpose of providing

announcements, promotional offers, alerts, confirmations, surveys, and/or other general

communication.

If you would like to stop receiving marketing or promotional communications via email from

RYKOV, you may opt out of such communications by replying "STOP" or by clicking on the

UNSUBSCRIBE button.

STATISTICS

LifelineTM may automatically gather general statistical information about use of the App and users,

such as IP addresses, browsers, pages viewed, number of visitors, browser type, computer or

device type, the website from where you navigated to the App, and the pages on the App that you

view, goods and services purchased/used, etc., but in doing so the App does not reference you by

individual name, e-mail address, home address, or telephone number. We use this data in the

aggregate to determine how much our users use parts of lifelineTM so we can improve the App. We

may provide this statistical information to third parties, but if and when we would do so we will

not provide any personally identifying information without your prior permission. We do not sell

or rent or share personally identifying information collected during your use of our the App other

than in connection with our conduct of the business of the App and health care without your

permission. We collect and store your profile in order for you to manage your health care and use

of lifelineTM

. The App was created to provide data analytics to you about your body and

health, including visualizations and trends of your specific data. The App accesses this data

with your permission. Acceptance of these Terms of Use of the App is your consent to utilize

this data. Your de-identified data will be used to recognize global health trends, better

understand diagnosis, diseases, and alternative care pathways including social support

systems to improve personal health via a holistic approach to better serve populations.

COOKIES

When you visit lifelineTM we send one or more “cookies” to your computer or other device. A

cookie is a small file containing a string of characters that is sent to your computer when you visit

a website. When you visit lifelineTM again, the cookie allows the website to recognize your

browser. Cookies may store unique identifiers, user preferences and other information. Generally,

we use cookies to: (i) remind us who you are and enable us to access your account information so

you do not have to reenter it; (ii) gather statistical information about usage by registered or

unregistered users and (iii) research visiting patterns. In addition, we use cookies to: (i) help target

advertisements based on user interests; (ii) assist our affiliates to track visits and process payments

and inquiries; and (iii) track progress and participation in lifelineTM activities. You can reset your

browser to refuse all cookies or to indicate when a cookie is being sent. However, some lifelineTM

website features or services may not function properly without cookies.

PROHIBITED ACTS

You are prohibited from violating or attempting to violate the security of lifelineTM, including,

without limitation, (a) accessing data not intended for you as user or logging onto a server or an

account which the user is not authorized to access; (b) attempting to probe, scan or test the

vulnerability of a system or network or to breach security or authentication measures without

proper authorization; or (c) accessing or using lifelineTM or any portion thereof without

authorization, in violation of these Terms of Use or in violation of applicable law. Violations of

lifelineTM administrators. Any unauthorized use terminates the permission or license granted by

lifelineTM. You may not use the lifelineTM logo or any other proprietary graphic or trademark as

part of a link or otherwise without prior express written permission.

COPYRIGHT AND TRADEMARKS

All pages within lifelineTM and any materials made available for download are owned or licensed

by RYKOV LLC. LifelineTM is protected by United States and international copyright and

trademark laws. LifelineTM and associated marks are registered trademarks of RYKOV LLC and

the App and website contain copyrighted and protected material. Some material in the website is

from copyrighted sources of third parties. Users of lifelineTM are solely responsible for compliance

with all copyright restrictions and are referred to the copyright notices appearing in the original

sources, all of which are incorporated by reference. As noted above, the content of lifelineTM may

not be copied, distributed, modified, reproduced, published or used, in whole or in part, except for

purposes authorized or approved in writing by lifelineTM administrators. If you would like more

You may download content only for your personal use for noncommercial purposes, but no

modification or further reproduction of the content is permitted. The Content may otherwise not

be copied, disseminated or used in any other way. If you violate any of these Terms of Use, your

permission to use the Content automatically terminates and you must immediately destroy any

copies you have made of any portion of the Content.

ACCURACY AND INTEGRITY OF INFORMATION

Although integrity and accurateness of lifelineTM is attempted, it makes no representations,

warranties or guarantees as to the correctness or accuracy of lifelineTM and Content. It is possible

that the information on lifelineTM could include typographical errors, inaccuracies or other errors,

and that unauthorized additions, deletions and alterations could be made to lifelineTM by third

parties. In the event that an inaccuracy arises with the App, please contact Support@lifeline.global

or access the “Contact Us” link at https://www.lifeline.global. If the inaccuracy is associated with

your data within your EHR and is imported into the App, please contact the respective Health

Information Management or Medical Records department at the hospital or physician office that

reported the information contained on the designated EHR so that it may be changed or updated

without notice. Additionally, lifelineTM shall have no responsibility or liability for information or

Content posted to lifelineTM from any health care provider or third party that is not a part of

trademark license.

GENERAL WARRANTY DISCLAIMER

RYKOV AND ITS ASSOCIATED STAFF DO NOT WARRANT THAT ACCESS TO OR USE

OF LIFELINETM WILL BE UNINTERRUPTED OR ERROR-FREE OR THAT DEFECTS IN

LIFELINETM WILL BE CORRECTED. LIFELINETM, INCLUDING ANY CONTENT OR

INFORMATION CONTAINED WITHIN IT OR ANY SITE-RELATED SERVICE, IS

PROVIDED "AS IS," WITH ALL FAULTS, WITH NO REPRESENTATIONS OR

WARRANTIES OF ANY KIND, EITHER EXPRESSED OR IMPLIED, INCLUDING, BUT

NOT LIMITED TO, THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS

FOR A PARTICULAR PURPOSE, QUALITY OF INFORMATION, QUIET ENJOYMENT,

RYKOV AND ANY THIRD PARTIES ON LIFELINETM ARE NEITHER RESPONSIBLE NOR

LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL,

EXEMPLARY, PUNITIVE, OR OTHER DAMAGES WHATSOEVER (INCLUDING,

WITHOUT LIMITATION, THOSE RESULTING FROM LOST PROFITS, LOST DATA, OR

BUSINESS INTERRUPTION) ARISING OUT OF OR RELATING IN ANY WAY TO

LIFELINETM, SITE-RELATED SERVICES, CONTENT OR INFORMATION CONTAINED

WITHIN LIFELINETM, AND/OR ANY LINKED WEBSITE, WHETHER BASED ON

WARRANTY, CONTRACT, TORT, OR ANY OTHER LEGAL THEORY AND WHETHER

OR NOT ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. YOUR SOLE REMEDY

FOR DISSATISFACTION WITH LIFELINETM, SITE-RELATED SERVICES, AND/OR

LINKED WEBSITES IS TO STOP USING LIFELINETM AND/OR THOSE SERVICES.

You agree to defend, indemnify and hold RYKOV harmless from and against all third party claims,

damages and expenses (including reasonable attorney's fees) against or incurred by LIFELINETM

arising out of your use or access of LIFELINETM, or access by anyone accessing LIFELINETM

using your username and password. If you have any basis for recovering damages from

LIFELINETM, RYKOV or their affiliates or third parties, you agree by your use of LIFELINETM

trademark license.

Some, or all of, these limitations or exclusions may not apply to you if your state, province, or

country does not allow the exclusion or limitation of incidental, consequential, or other damages.

RELEASE

Because we are not involved in the rendering of medical advice, in the event that you have a dispute

with any individual and/or entity that arises out of your use of lifelineTM, you release lifelineTM

and RYKOV (and our officers, directors, agents, subsidiaries and employees) from claims,

demands and damages (actual and consequential) of every kind and nature, known and unknown,

suspected and unsuspected, disclosed and undisclosed, arising out of or in any way connected with

such disputes. If you are a California resident, you waive California Civil Code §1542, which says:

" A general release does not extend to claims that the creditor or releasing party does not know or

suspect to exist in his or her favor at the time of executing the release and that, if known by him

or her, would have materially affected his or her settlement with the debtor or released party."

ONLINE TRANSACTIONS – PAYMENT OF BILLS

Users are able to pay for products and services online. Our financial services provider manages

and operates the online bill payment feature, including the processing of credit card, debit card and

e-check bank accounting withdrawals.

If a credit card account is being used for a transaction, RYKOV on behalf of lifelineTM may obtain

preapproval for an amount up to the amount of the payment. You represent and warrant that if you

are making online payments that (i) any credit card, debit card and bank account information you

supply is true, correct and complete, (ii) charges incurred by you will be honored by your

credit/debit card company or bank, (iii) you will pay the charges incurred by you in the amounts

posted, including any applicable taxes, and (iv) you are the person in whose name the card was

issued and you are authorized to make a purchase or other transaction with the relevant card and

card information.

TRANSFER IN THE EVENT OF SALE OR CHANGE OF CONTROL

If the ownership of all or substantially all of our business changes or we otherwise transfer assets

relating to our business or lifelineTM to a third party, such as by merger, acquisition, bankruptcy

proceeding or otherwise, we may choose to transfer or sell your personal information. In such a

case, unless permitted otherwise by applicable law, your information would remain subject to the

promises made in this privacy statement unless you agree differently.

SECURITY OF YOUR INFORMATION

When personal information is transmitted from your personal computer and/or mobile devices to

lifelineTM, it is protected in transit through the use of encryption, using the industry-standard

Transport Layer Security (TLS) which has replaced the Secure Socket Layer (SSL) protocol. Our

Our Transport Layer Security software is the industry standard and among the best software

including credit card number, name, and address, so that it cannot be read over the

internet. However, since wireless networks, personal computers and the Internet are NOT a 100

percent secure environment, we cannot guarantee, ensure, or warrant the security of any

information you transmit to us or provide to lifelineTM or to health care providers and which is

viewable via lifelineTM. As a result, while we try to protect your personal information, you access

and use lifelineTM at your own risk. There is no guarantee that information may not be

accessed, disclosed, altered, or destroyed by breach of any of our physical, technical or

administrative safeguards.

It is your responsibility to protect the security of your personal computer and devices, username

and password. As a user of lifelineTM you hereby agree that you (and not RYKOV or any lifelineTM

associates, officers, staff, or the health care providers) are solely liable for viruses, worms, Trojan

horses, malware, ransomware, bots, and other electronic mechanisms designed to destroy or impair

the functioning of computer systems and residing on your personal computer and/or mobile device.

Our personnel will never ask you for your password in an unsolicited phone call or in an unsolicited

email. Remember to sign out of your account and close your browser window when you have

finished your session. This action will help ensure that others cannot access your personal

information and account information if you share a computer with someone else or are using a

computer in a public place where others may have access to it.

CHANGES TO TERMS OF USE / ONLINE PRIVACY

LifelineTM reserves the right, in its sole discretion, at any time, to modify, alter, or update the terms

and conditions of these Terms of Use and online Privacy & Security Policy with or without notice.

Modifications shall become effective immediately upon being posted on lifelineTM. You should,

therefore, periodically visit the website and/or app information to review the current Terms of Use,

so you are aware of any such revisions to which you are bound. Certain provisions of these Terms

of Use may be superseded by expressly designated legal notices or terms located on particular

pages within lifelineTM. Your continued use of lifelineTM after modifications are posted constitutes

an acknowledgement and acceptance of the Terms of Use and modifications.

In the event that any of the Terms of Use are held by a court or other tribunal of competent

jurisdiction to be unenforceable, such provisions shall be limited or eliminated to the minimum

extent necessary so that these Terms of Use shall otherwise remain in full force and effect. These

agree to resolution of such dispute in the state or federal courts located in Georgia, in accordance

Security Measures

Protecting your confidential health information is a top priority at RYKOV. In addition to applying

stringent confidentiality policies that govern access and use of information by RYKOV personnel

and staff, we have implemented security features and methods to protect your data in lifelineTM

including the following:

o Multi-factor Authentication — We have not yet enabled the use of multi-factor authentication for lifelineTM. In the near future, this will be provided by default and you may

choose to opt out of multi-factor authentication. The data you enter at this time is you’re the

data you as a user control.

o Encryption — We encrypt the communication from your Web browser to our secure Web

server using HTTPS/TLS, and data at rest is encrypted by default.

o Firewall — Personal health information is stored in the respective secure her, and EHR

system, and through secure lifelineTM databases, protected from the Internet by a firewall.

Our services are protected by firewalls and access controls to restrict unauthorized

access.Policies and Procedures — We maintain internal policies and procedures that limit

access, use, disclosure, and retention of your information by our workforce. We follow role-

based access controls (RBAC) to limit workforce access and maintain internal policies for

secure data handling.Session timeout — After a period of inactivity, the logon session will

time out. We utilize session timeouts to ensure inactive sessions automatically expire.

o Monitoring — We monitor the Web server for evidence of unauthorized break-in attempts.

We will apply the latest security patches. Further integration work is in process of

development to leverage the monitoring tools at lifeline's disposal. Prior to your accessing

third-party EHR data and clinical data from your health provider, we will have a monitoring

system in place and will update the terms herein.

Revisions to this Privacy and Security Policy

As state and federal laws change, and as we add new features to our Web site, RYKOV may

periodically revise this Privacy and Security Policy. Any revision of this policy will apply to all

information we already have about you at the time of the change and any personal information that

is gathered about you after this time. Any future updates to this Policy are effective as soon as they

are published.
